Do authors really write their book or do they just feed off their ideas to ghostwriters or something?

I see these celebs such as Lauren Conrad who I have hard time believe she wrote the book..she probably just said her ideas and someone wrote it for her...a ghostwriter maybe...she just doesn't come off as someone smart enough to do that and her being a celebrity who makes money off doing a reality show makes it hard to believe
