Should I move on?

I have been in a LDR for almost 4 years. I really thought he loved me. I was hospitilized with a serious illness a few weeks ago and spent 3 days in ICU and then 3 more extra days in the hospital. He didn't one time come and see me. He drove here Christmas Eve and left my presents on the doorstep pretending to be Santa thinking it would cheer me up. Hello, I needed him not presents. I feel like our love was put to the test and it failed. I am so very hurt and don't know what to do.
