9 mth old with high fever?

9 mth old doesn't run fever with ear infections. found out 4 days ago she has double ear infection. been on amoxicillin for the past 4 days and suddenly at 4pm she is running 104 fever, vomiting, and coughing. very lethargic and not eating or drinking. gave her motrin and now fever is down to 102. have cold compress on her head. any thoughts before i take her to the er?
